dependet visa

hello guys,

I need some information please , regarding dependents visa..

current situation is , I have already received stamped Visa for Tier-1 (did work permit to tier 1)

1) Need to apply dependents visa (spouse and kid) , i hope i need to send 2 dependent application
form(1 kid , 1 spouse) - question is do i need to pay total of 395 for application fee or it
would be 395*2

2) In dependent application at one place dependent has to sign (page 24 of 33 in application form in declaration
section ), how about for kid below 18 years ? i didn't see section for authorisation for kids below 18 years
