Referee declaration form

I am applying for citizenship on the basis of having ILR, having lived in the UK for the part 3 years, and my wife being British (also naturalised). In the document checklist it asks for "Two referee declarations", but does not provide a link to the declaration form.

Looking online, this seems like it's the correct form.

Can someone confirm this is the form my referees should sign, or is there another one somewhere I missed.

They are part of the online form process and downloadable at the end.
