Sponsor transfer Dependant in foreign country

I am on Tier 2 Visa and I have accepted another job offer and my dependants are in Pakistan. I have below questions:

1. Do I need to apply for transfer of sponsorship together for all including myself and all dependents

2. Do all of us need to be in UK and my dependents are overseas at the moment and planning to return after more than a month and obviously employer will not wait till then.

3. Is it possible that I get appointment in UK and they get it done in Pakistan.

4. If we all need to be at same location, can we do it from Pakistan.
