Switching from WP to Tier 1, has HSMP approval letter

Hi Friends,

I need clarification on the following please?

1) D3. When was the entry clearance or visa issued?

On my EC there is no issue date, it contains only Valid from and Valid Until dates. Please let me what should I write here?

2) D4. Please enter the reference number on the entry clearance or visa within the applicant’s passport or travel document

Which number should I write here as there is no reference number in the EC. There is one number at the top of EC in red color font and a VAF number.

Please send me your suggestions, eagerly waiting to apply soon.
