Tier Dependent IHS asking Work start date and work End date

Hi,

I am on ILR and applying for my wife's Tier2 dependent extension.

In the IHS(Immigration Health Surcharge) application it is asking me to enter work start date and work end date.

I no longer work for the company that sponsored me my tier2 visa.

What should I need to fill in these fields?

During my last extension they did gave me start and end dates, should I need to fill them or my wife's visa end date as start date and I am applying to 3 years so 3 years from then as end date?

Any suggestions are really helpful.
