FLRM Wife's visa is up for renewal in 2 weeks

Hi Guys,

I am stressing a bit trying to put together all the information needed for the FLR(M) application. So if you could advise I would grateful.

So My queries are as below:

1. My wife entered UK on 29/12/2018 and her visa is expiring on 20/09/2021 so what date should we put in the application so as to follow the ILR rules later on.

2 As per the cohabitation requirement we need to provide correspondence for the last 2 years, please could you check if I have this correct.

1. Aug 2019 Joint Bank
2. Dec 2019 Joint utility
3. Apr 2020 Phone bill (husband)
4. Apr 2020 Phone bill (wife)
5. Aug 2020 Joint Bank
6. Dec 2020 Joint utility
7. Apr 2021 NHS Letter (Wife)
8. Apr 2021 Broadband bill (Husband)
9. Aug 2021 Joint Bank
10. Sep 2021 Joint Utility

3. We are also living with our parents so do we need to provide a affidavit of some sort for stating that we have permission from our parents to live in the house.

4. We had a baby last year, do we need to provide information for the baby to strengthen the application?

5. Housing report was submitted during the first application, however from what I have read this is not require during extension?
