lost passport with workpermit. urgent

my brother has a pakistani passport with the work permit inside (the entry clearance page) but has lost it. he willl get a new pakistani passport but how will he get the new workpermit on the passport and how much will it cost?

also he has lost the original work permit (the a4 certificate type thing)
