For first time naturalised child passport application, is there any document required apart from:
(1) Original child birth certificate
(2) Original child certificate of naturalisation
(3) Cover page printed out from passport online
(4) Child Passport Identity and Consent Form printed out from online
Any other document required?
Secondly, i'm not sure what to tick in the attached as there is no option of "NATURALISED"
So what should i tick there? Confused
