IHS surcharge fee showing for NHS Health care work

Hi, I'm due for my tier 2 visa extension . However when I tried to do my application online, on the section of IHS payment, it gives me 3 years total payment summary. As of I'm aware, nurses and their dependants don't have to pay it as it was taken off. Is it because that I did something wrong in my application? Could you please help me with this?

There is a question on the application form asking if you are eligible for Health and Care visa. You should answer yes if you are eligible, to not pay IHS
