Dependant HSMP Extension

Currently I am in on HSMP since March 2007 and got my wife here as my dependent in July 2007. Now I have to apply for the extension.

I am looking the answers for the following questions. Will really appreciate if any one can help me

1. How early i can apply for my extension ( 1st HSMP expiring in FEB 2009)
2. Will I have to apply for my wife as well with my application or it will be a separate application for her
3. How much fees i will have to pay ? ( for myself and for my wife )
4. Last but very most important question : What if my wife needs to go back to her home land for 1 year .. should i apply for her extension first or i can apply for her visa anytime in future from her home country
