Tier 2 (General ) visa - Extend

Hello

I am planning to extend my Tier 2 (General) visa with my wife & a child
there are living with me in UK. I am with same employer, same job code and earning more than ongoing rate £31800 (job code 2139)
1- could you please advise - what documents I need to submit for extending visa
2- Does my Employer need to apply for new restricted COS or I will be using the one I was given three years back.
3- is there any other things my employer need to do or i should request from my employer
4- any finance requirement (maintenance or any other)
