I recently got my Irish citizenship through the FBR and while I'm waiting for that certificate to arrive (along with the rest of the documents I sent) I want to get things ready for my passport application.
I'll be applying from the US, so I can use PassportOnline.
What documents to I need? I'm going in circles here and need clarification.

What Documents do I Need?
1. Your full civil birth certificate
[Easy, got it.]
2. Your civil marriage or civil partnership certificate (if you have changed your surname)
[have it but don't need it as my name hasn't changed]
3. Proof of address

The links for proof of address and proof of name don't give any useful information. When I follow the path, it ends at "you need to get your FBR" but I already did that.
So what do I need for Proof of Address and Proof of Name?

These two also have me confused. I don't have a Public Services Card. So 5 seems to be redundant with 6.

I am also from the US and just got my passport about a week ago.
Here is what I provided:
Birth certificate
Certified copy of my US passport.
Proof of address:
Bank Statement, Cable bill, I also provided a downloaded copy of a pay slip from my company's payroll port, ADP.
Proof of Name:
Bank Statement, Cable bill (not the same ones, but the month prior.) I also provided a downloaded copy of a pay slip from my company's payroll port, ADP.
All pretty straight forward. I provided 3 items for proof of address and name, just to be safe. They were all downloaded from the online portal (I don't get anything in the actual mail.)
One thing that got me is, I didn't want to send my actual passport so I sent a certified copy. The first time I had it notarized the notary (at a shipping store) did not sign and date the actual Identity verification form, but rather provided a separate form that he signed and dated. This is legally how they are supposed to do it for US documents. When my application was being processed, the identity verification form was not accepted.
I took the form back to a different shipping store, explained the issue to the notary. She confirmed that the previous notary had followed proper procedure, however since the document was not for US purposes but rather being sent to Ireland, she signed it, dated it and stamped it etc directly on the form.It was not a big deal to me, but it did delay my application by about 2 weeks, as it took about 10 days for the new document to get to Dublin from California. The point being, if you're using a notary, make sure they sign and date and stamp the actual identity verification form.
The process was pretty fast, including the time it took to send the new document, it was less than 5 weeks from when they received my documents to me receiving the passport and passport card.
Be warned tho, registered mail from the US to Dublin takes a long time and was the longest part of the process.
