Questions regarding self-employed applicant

The application requires upload of:

most recent tax year account statement and corresponding HMRC tax return as proof of self-employment

I have the tax return but what is the tax year account statement?

Also, it asks for employer's name/business name. I don't have a particular business name, so should I just put my full name here?

I have the tax return but what is the tax year account statement? I think called the SA302 which you can obtain/print via HMRC Online - https://www.gov.uk/sa302-tax-calculation

Documents are uploaded after you have submitted and paid and are directed to the UKCVA website for documents and appointment booking.
