Evidence of cohabitation covering 2years preceeding application date from at leat 3 sources spread evenly 3-4months interval.

The questions in Appendix is already incorporated into the online form so you don't need to fill it separately.
If yes please send me link of appendix form.

Thank you for your reply.
I worked with my employer since 2019 and get paid every 4 weeks .
6 payslips 4 weeks total only 24weeks . I have to add one more payslip will 28 weeks .
My last payslips show before tax:
23/12/23. £1900
20/01/23 £1900
17/02/23. £1900
17/03/23 £1900
14/04/23 £1900
12/05/23 £2040
Next one 09/06/23 around £2000
Do I have to calculate like this:
add total :
£13540÷28weeks= £483×52=£25145
Thank you

More or less that's how you calculate it to annualise. Note that you will need to provide each payslips and account statements for last 6months.

Can't say if mandatory at extension of Spouse leave but hardly know of an application going without it.
If lost then request another from landlord or property manager, May come at a fee.
