Forget to submit family consent form

I forget to upload family consent form while submitting visa extension application. After submission, I can’t find option to upload and it was mentioned as mandatory document.
Is there a way to send these forms to Home Office. What shall I do now. Any help please.

Have you attended biometrics appointment? If not take it along to your appointment for it to be uploaded for you.
Previously, CW requested applicant to provide it.

I have attended biometric appointment and requested the staff to scan and upload these consent forms but the staff replied that you already have submitted all the mandatory documents and if the home office need these then they will enquire through email.

I would think staff should have uploaded it for you at the appointment.
The optiin given is in line with my earlier response. CW can request you to provide it when your application is beingbattended to.
