Unenrolled deed poll for name change Indian passport with no last name

Those who have changed/rearranged the order of their names via unenrolled deed poll, can you share the template that you have used? I see a template in gov.uk website but note that there is a para on informing police. Is that a must to include in the deed poll even if it’s not applicable in my case?

That template says, where applicable, so you can include it, for example, it may be applicable if you were a registered offender who needs to report name changes to the police.

Can it be on a plain white A4 paper and does it need to have any title etc or can it start directly with the declaration?

Plain paper is ok. You can just copy the sections that apply to you.
