Dependent skilled worker application

Hi everyone,,please need urgent help.
I just have filled dependent visa for my child(10months old)born in uk and at the end of the application i got 2 form in mendatory action 1 is parental consent i got understanding of tht and 1 is verification check that got 3 parts.
Iam not sure whether to fll that form or not please help me out-
i already have got my visa.
