RENUNCIATION OF INDIAN CITIZENSHIP

Hello,
We got our British Citizenship in 2011, ( wife, 2 kids, and myself) after we got our UK passports we surrendered our Indian passports and got the surrender certificates, wife and I applied for OCI back in 2012 and got it, and ever since we traveled to India with OCi and got evisa for children as they were minors, now my children are over 18 and my daughter applied for her OCI and was told she need a RENUNCIATION OF INDIAN CITIZENSHIP proof, Wife and myself never applied for it, so while trying to apply for my daughter it says

THE CIRCUMSTANCES IN WHICH/REASONS DUE TO WHICH APPLICANT INTENDS TO ACQUIRE FOREIGN CITIZENSHIP AND RENOUNCE INDIAN CITIZENSHIP

The Circumstances in which or reasons due to which the applicant intends to acquire foreign citizenship and renounce Indian citizenship

When words like intends to acquire then how is it applicable to someone who has already acquired citizenship of the UK?

Also, the form needs the name and nationality of the Father and mother details will they require the the parents to apply for the same?
