Unenrolled deed poll to move surname to surname field

Hi my current passport doesn't list anything for my surname and all three parts of my name are listed together under my given name.

In the template for an unenrolled deed poll, it says

“I [old name] of [your address] have given up my name [old name] and have adopted for all purposes the name [new name].

My name doesn't really need changing, only rearranging; how do I make this clear in the deed poll so BP can be issued with the correct name order?
