FLR extension partner of ILR holder

Wife has ILR, partner in UK for 3.5 years, current stay expires in Sept 2024. What are the options for extensions? FLR(M)?

Depends on what visa spouse currently has, that's about to expire?

Is it FLR M I use to apply or as skilled worker dependent. As the skilled worker dependent application asks for Cos number?

It is skilled worker dependent form and you put the last cos you had before ilr.
