Query on Contact Details

Hi All,

Can anybody please guide in Part 4 - Your contact details, of VAF9, what address one can give.?
In 4.1 I want to give my native address and on 4.5 I want to give my current address (which is a rented accommodation). And in 4.2 Home (landline) telephone number, Can I give my native home's land line number?

In my passport also my native address is given.
