HSMP(Tier 1) Genreal Extension+ Dependant Visa Advise

Hi to everyone. Just wondered if anyone might be able give me some advise on my current extension.
My HSMP expires Mid March 2009. I recently got married in January and plan to apply for my husband to come as my dependant. We were really hoping we could go down the shortest route for application, as having to be appart as newlyweds is a bit miserable (plus im hospital doc, so no time even for phones etc..)

So the question is do I:
- Apply for extension(switch) to Tier 1, then apply for a dependant visa for husband
- Apply for extension and dependant visa same time.

I tried to find some info on the ukba website, and even previous posts on the board, but cant seem to find the answer.

Most grateful for any tips!
