How to make an affidivit.

Dear friends,

Am from Bangalore. In my first company payslip my name is mentioned as "Rajbir Singh" while in the second company it is mentioned as "Singh Rajbir". Do i need an affidivit to tell that both the names are same.
Can anybody tell me the steps required for an affidivit, and what documents i should have for that.

I don't think it is required as it is quite obvious unless there is change in spelling etc. You should be fine.
