Surname change after marriage, queries...

I am filling application form (extension) which asks


"last name(s) or family name(s) as stated in the dependant’s passport:"

After marriage my passport is amended, now my new surname is mentioned in amendment page (5th page of p.p.) but the main front (with picture) page has still got the old surname.


Which surname I am supposed to use for above question?

note: married in london few years ago and we have marriage certificate

Please suggest if any one is in same situation and/or have solved the problem.

You should mention the new name. It doesnt matter if the front shows your old name because as you say there is an amendment on the inside page. you are going to send your marriage certificate, so that will clarify the situation further.

Did you receive your extension after the name change?


I have query since you have already completed this process. Would you please provide advise?

My wife wants to change surname to new surname (of mine, that is one after marriage) but her hsmp visa is still on old name (that is maiden surname). Will this affect her visa if she gets observation on passport on a inside page and not on the first page itself?
