Birth Affidavit Format for a minor/ adopted child

Is anyone aware of a format for a birth affidavit for a minor or, specifically, for an adopted child? I do not have a Long Form Birth Certificate for my Chinese-born adopted daughter and am applying for her naturalisation, and am going to be making an affidavit to accompany the application. I want to be sure to use a format acceptable to the DoJ.
