Application form for tier 1 general

Hi,

Where can I find the tier 1 general application form to apply from India?
also is there any format of the letter we need from our employer showing our details?

thanks can I also get a format for the letter we need from the employer please?

also do we get more points for MBA?I saw this on the website..
MBA 75 points
(only for applicants who enrolled on an eligible MBA course before 30 June 2008 and who graduated within 12 months of submitting their application. More details are in Annex D of the Tier 1 (General) Guidance)

Only for the applicant who has completed MBA from the elite institutions mentioned in the guidance notes.

The list of MBA institutes with higher ranking is given in the guidance notes starting from Page 46.
