VERY URGENT HSMP Extention Dependent Application

Hi Guys,

I would appriate if someone can reply asap.

I am submitting my HSMP Extension non JR to Tier 1 along with dependent application.
(1) Do I/my dependent need to provide marriage certificate (It was presented at the time of my dependent's entry clearence)
(2) I will be paying by card in PEO. Do I need to fill card details of leave it empty or tick cash (for both applications).

Sorry I think I havent put the question properly. 'Obeviously' means do I have to fill or to leave empty!!

Marriage certificate is not mandatory for extension application if your dependant's passport contains your name as spouse. I have not submitted the certificate for my recent extension application along with dependants.
