Workpermit to Permanent Residence in UK

Hi,
I have been in UK more than 3 years 08 months on WorkPermit and working in software company. Now I like to apply for UK Permanent residency.
Please Could anybody help me for documentation details.

You can get the forms from the following website:

www.homeoffice.gov.uk and seleting the immigration and nationality section. The form you want is SET(O), do note that you can only apply after 3 yrs and 11 months.
