I am sending bank statement as proof salary evidence and also for funds evidence should I add the addess of my branch where I opened my bank account and also the name and designation of bank manager or this question is asking for something else?

Just provide the bank name and the bank's phone number (which should be printed on your bank statement).

You need to provide the contact details (Name of person, organization, mobile/ph & email ids of:

1. Manager of ur company for checking the earnings and employment verification
2. Accounts/third party pay roll division if it is the case
3. Bank Managers details in case if u r applying from outside UK
