Hurray.......GOT HSMP Extension under Tier 1

Post by dnshah2k » Wed Jul 22, 2009 8:59 pm

I alongwith my 3 dependents got HSMP Exension under Tier 1 approved for 3 yrs. and received the documents yesterday

Thanks to Push, nskg, uk dreamer,& bitterweb for all your guidance and support

Now atleast I can say my case was straight forward, I had submitted following documents which may help others in their application

Cover letter with Claim Sheet-defining points claimed and basis of claim
Self assessment printout
Original HSMP Letter
12 Months pay slip
12 months current account bank statment - for Previous earning and maintenance fund requirements for me and my dependents
Marriage certificate
Birth certificate

Extra Document- Councli Tax Bill

I haven't enclosed the "Degree Certificate" as it was the same Qualification of initial HSMP application

I'm glad to share my format of cover letter and claim sheet if anyone needs for their application as FORUM has developed the confidence of submitting my application on my own
