Visitor Visa rejected -- 10 yr ban

Hello all,
My friend applied for a visitor visa in India 3 times ( all of them within a span of one month) and rejected. First two times were based on the fact that his income proof were not satisfactory for the entry clearance officer. He is one of the directors of his own business, so his salary credited into the bank account had to be proved, as they did not have salary slips.
On the 3rd occasion, in the application form where it asks for "Have you ever rejected a visa to any other country including UK", he had mentioned YES and given the 2nd rejection reference number and date. He had also included a covering letter that he was denied visa twice along with the reference numbers, along with the other necessary explanations and document lists etc. But this time they rejected the visa and applied a ban for 10 years, saying that "he had failure to disclose information" as he had not mentioned the first rejection's details.
How can they say it is failure to disclose when he has mentioned it in the covering letter and also latest of two rejections in the application form? Is the ban legitimate or is it case of mistake by the high commision?

Don't think this is a legitimate ban. It can surely be challenged.But i am not sure how?
