self employed,please help!

hi there,my wife is on self employed now as an Eu citizen after she lost her job in January this year. October could have been her one year of continous employment in the uk which qualified her for Certificate of Registration.We are about to make a non EEA family member application but dont know which application form to use as a she is a self employed. Thanks
