here Printed name = Full name in CAP's)

I am seeing the application from page 35.. (declarations )

1. In the name blocks - put ur dependents name in CAPS
2. In signature box you sign.. and above it in brackets put (For ur child's name)
3. under the signature box.. i wrote my name in cap's

regarding HSMP thing.. i would wait for experienced guys to reply as i converted from WP...
