This shouldn't really be a problem as long as you make it very clear on your cover letter and elsewhere that you are submitting this statement for 14/06/2009 to 14/09/2009
However since you are applying for dependants as well, you need to have £533 for each dependant as well.

B26/27/28 are for people who have lived in Northern Ireland only, so yes if you haven't lived there you can leave it blank.

You don't need to submit either of these for an extension. Just mention if your cover letter that you are not submitting these as you previously satisfied this requirement through your initial HSMP application. (mention your HSMP Reference Number).
I did the same thing and it came through with no problems.

Mention both. There's never any harm in mentioning something like this, its understood that women change their surname after marriage, but there's no harm mentioning the dates either.

Put the photos in the same envelope. This is what the form says on Page 10:
Please place the photographs in a small sealed envelope, together with those of any dependants included in section F, and attach it across this space
Make sure you write your names on the back of each photo. On the back of your spouse's photo also mention that she is a dependant of yours.

Send it by Royal Mail Recorded Special Delivery. You can select the Next Day Guaranteed before 1 pm option.
You can send a self-addressed pre-paid envelope along with the pack as well, so you receive this quicker.
Both of these (if sent by Recorded and Guaranteed) will cost you £13.40 as long as your application weighs under 1 kilo.
Go to the Post Office before 4.30pm as sometimes the last postman visits the branch around that time, and then your package won't leave till the next day.
There is no best day to send the application, if you send on Monday it will reach Tuesday and so on. If you send on Friday, then ask them to deliver it on Monday.
