Thanks to the help on the forums I have submitted my application today. Here is my cover letter and the list of documents

1. Age Assessment - 20 points claimed

Documents provided:
i. Original Passport

2. Educational Qualification - 35 points claimed

Documents provided:
i. Original Masters Degree Certificate from University of xxx
ii. Original Transcript from University of xxx

3. Past Earnings - 15 points claimed

I am submitting my bank statements from xxx and xxx for the period along with my payslips, employment letter and salary breakdown as supporting documents for my previous earnings from two separate verifiable sources. For my second earning source there is a letter from the customer, Accountant and the bank statements.

i. Weekly pay slips from 18th of Sep, 2008 to 17th of Sep, 2009
ii. Letter of Employment
iii. Letter from Business indicating Freelance project arrangement.
iv. Accountant’s Letter for Contract Self Employment
v. Bank Statements for the claim period showing my Net salary (marked with pencil in double lines) & Project earnings (marked with pencil in single lines) credited into Bank.

4. UK Experience - 5 points claimed

Documents provided:
i. Weekly pay slips from 18th of Sep, 2008 to 17th of Sep, 2009.)
ii. Letter of Employment

5. English Language Requirement:

Documents provided:
i. Original Masters Degree Certificate from University of xxx
ii. Original Transcript from University of xxx
iii. IELTS Certificate


6. Maintenance Fund:

Documents provided:
i. Bank account statement showing a minimum balance of not less than £800 from 16th of June, 2009 – 16th of Sep, 2009. The bank statements are from the same account as used for the salary and freelance.

7. Additional Documents
Documents provided:
i. Original Work permit document
ii. Print out self-assessment form from the UKBA Home office website.
