HELP: section D in tier 1 general HSMP extension form

PLease can someone help anwer my question?
1) came to the UK on HSMP in Nov 2007.
2)Entered the UK once before in Jun 2007 on visitor visa.
section D is very confusing to me?
any help?

The answer is November 2007 since tourist/visitor visa does not give you right to live in the UK.

thanks! so basically, fill out all of the information from nov 2007

one mre thing, I got a copy of 12 month statements from barclays however they have not signed, stamped or no logo..although they have given a cover letter saying that it is all accurate. is that enought?
pls could someone let me know?
