PBS: TIER 1 (GENERAL) DEPENDANT

Hi,

We got married on 23/3/2009 and my wife got 'Tier 1 (General) Migrant' under Point Based System on 14/4/2009. Now I'm planning to apply for the 'Tier 1 (General) Dependant' Visa next week, as we are planning to fly next month.

Since she is still in India, what all documents are required to submit and which form needs to be filled for this visa?

Can you also assist me with a sample cover letter for application & for bank?

I have another question for you. Since my wife (main applicant) is still in India, do my cover letter for the dependant visa will be represented/addressed by my wife (main applicant) or by me (dependant)?
