I have operated Limited Company and I have taken suffficent salary required for HSMP.And Now Iam pernanent employee.

In summary, Iam showing 7 month salary slips from my limited company and remaining 5 pay slips from my pernanent job.

So which category I fall under. is it self-employed Apllicant or Employee.
And also can you guys tell me what are the document I need to produce under each category.

You will claim two earning sources:
1- Self-employed (ccompany): required documents (letter from accountant + bank statments).
2- Employee: required documents (payslips + bank statements)

And also can you guys tell me what are the document I need to produce under each category.

take care that u have paid PAYE from yr company , it is subject to yr salary

but as u mentined that u got 12 months payslips (part emp+ part comp)
