HSMP(2yrs) to TIER1(3yrs) extension confused

Dear all,

Like most of the HSMP visa holders I am confused in regards to the extension procedures. There's vast information online for each individual case and I couldn't deduce the right information with regards to my application. I am confident that I can find the correct answer in this forum.

Here's my scenario:

1. I applied HSMP under the qualifying criteria set on 05-Dec-2006.
2. Received HSMP approval letter on : 15-Dec-2007
3. Initial HSMP Visa for 2yrs received on : 03-Jan-2008
4. Expiry of Initial HSMP Visa : 03-Jan-2010


So friends, could you please advise me the following :

1. Where can I find the right application form and guidance notes.
2. Right application form for my wife's extension as my dependant.
3. Fee for myself and my wife.
4. Any known exemptions in my case.

if you are appliying dependent along with the main applicatnt at the same time

the fee will be:

Main applicant £820
