Dependant Visa - Immigration History

Dependant Visa - Immigration History

Hi,
I came to UK on a work permit and later my spouse & son joined me as my dependents.
After sometime, I got a HSMP and now I'm planning to extend it (as Tier 1 General).

In the Dependant application form, Section D (Immigration History) has the following questions

D3: What date was the entry clearance or Visa valid from?
What should I give? Is it-
a) "Valid From" date mentioned in the work permit or
b) "Issue date" mentioned in the HSMP or
c) The date they entered UK (the date stamped during immigration)?


D5: Enter the reference number on the entry clearance or visa which can be found in the dependant's passport or travel document
Which reference number should I provide? Is it the work permit one or the HSMP one?
