It would help if you copy paste the question also..

H1. What is the dependant’s relationship to the main applicant?

I think you should tick SPOUSE and go to Section J as she is your wife (married)

Could you or someone also assist regarding the following questions as well,

I am applying for tier 1 g, in dependant form for my wife. There is a question in the begining of the form where they ask what is your name according to passport and also ask if you were known by any other name. Before merriage my wife had different sir name and after merriage it was changed to my sir name and on her passport she is got my sir name.

Now if we mention her previous name then what kind of evidence is required, we have merriage english translated certificate from pakistan. Will that be sufficient? Or we need ti have it translated again from uk? Urdu Merriage certificate is certified by the foreign office as well as pakistan embassay.

My second question is, when she got her post study work dependant visa from pakistan, her finger prints were taken. Do i need to mention it at the form.
