how to get information

hi every one.
i m planning to put my tier1 file from india.
my case is beet complicated.I been to uk from last four year.
First visa i got was WHM from dec 2005 to nov 2007
Second visa was from Jan 2008 to June 2009
Current visa STUDENT DEPENDENT TILL OCT 2010.
Now my querry is when my whm visa near to finish i meet one local agent and i had given all my documents for hsmp visa i also paid lots of money even i given him the home office fees in cash.At that time i got rejection and i dont have any documents for that.I not trust on that agent as i got rejection in ten days.So i m just worried when i have to fill the form for tier 1.
How many time i applied and how many rejection i got.
Could any one help me how i can ask home office about my querry.
I hope i explain properly.

request your SAB report from UKBA. That will have whole of your immigration history plus the details of your last application made via agent.

Is that any special form or procedure i have to follow for that.
As i have not any idea about sab report.
Is there any specific charge for that.
