Fill it with Pen in Black Ink. The following is from the Section-1 of the application form but there is no harm in following it for other sections as well:
Please complete this section in block capitals and black ink.

Hi,

Can you also advice on the following, i found these notes on home office website

What services are not available?
Applying in person

This page explains what our public enquiry offices cannot offer, and categories of people who cannot use their service.

The offices cannot consider your application if you are:

â– a person with exceptional leave to remain;
â– a person with discretionary leave to remain or who has been given humanitarian protection;
â– applying for permanent residence on the basis of 14 years' residence;
â– applying for an extension of your visa on the highly skilled migrant programme;â– a business person, innovator, sole representative or investor;
â– a seaman;
â– a Gurkha; or
â– an elderly dependant.

I am on HSMP and need to extend my stay, visa expiry in march, and it was grated in 2008 for two years.

I have booked my appointment at PEO with PBS Tier 1

can you please advice

Although you are extending your stay under HSMP, it will be done under PBS which is an eligible VISA category for "application in person". HSMP does not exist any more.
