Summary sheet PBS Dependant Form

Hi,
I need help on On summary sheet part A for PBS Dependant Spouse form:

Passports - YES ATTACHED

Police Registration - N/A

Marriage Certificate - YES ATTACHED

Birth Certificate - N/A

Driving Licence - ????

He got driving licence but have applied to change the address does it necessary to attach driving licence can you please advice

Please mention it as YES if your dependent has used it as evidence/proof if not N/A is fine

But i want to know why do we need to submit Driving Licence, we will be providing Original Passport,

Is that requirement, because i have not read any notes on providing Driving Licence, i think this is the 2nd option for ID if passport is not provided.

I am not sure can you guide please

Can you also confirm if i am using the correct form for Extension, I am on HSMP the visa was issued in 2008 and i need to extend it.

I know it look stupid but i just read a topic of getting rejected because of wrong form.

You are right you can leave that blank...

Yes you areusing right form for Main applicant.. for dependent pls seethat the lastest form is updated as 15th jan 10
