Help needing regarding filling the form

Hi all,

I am currently on HSMP and now will going to extend my stay under Tier1(General) for another three years. I come across difficulty in filling the form. Can someone tell me what do I need to put in for

- B10. Home Office reference number
- B11. Give details of any current or previous worker reference number(s) under work permit arrangements
- B13. Points Based System migrant reference number
- B14. United Kingdom ID card number

Any help will be highly appreciated?
