Queries about dependent application form (Tier 1 extension)

Hi,

I am in process of applying for Tier extension (Switching from HSMP) and would like to clarify some doubts about Dependent application

1. What should I fill in 'Home Office reference number(s)' (Section B11)
If I dont have/remember this, can I leave it blank.

2. What should be filled in 'Points Based System migrant reference
number'? (Section B14)

3. What should be filled in 'What is the dependant’s current immigration
status in the United Kingdom' (Section D12)

3. HSMP Child (It will be written on the Visa of your child)

For the dependant's current immigration status, it will be HSMP Partner or HSMP Child depending on the dependant.
