Include your birth certificate though I think passport might be sufficient. Hope your bank statements covers the period you are showing as evidence for earnings and not just the last 3 months which you are using for maintenance funds evidence.
Take photocopies of your original docs and retain for yourself for safety.

5.3 Spouse/Partner’s date of birth
5.4 Spouse/Partner’s place and country of birth
5.5 Spouse/Partner’s nationality
I hope i do need to fill in only these applications with proofs.
Please spare some time and guide me.
