request for tier1 dependent and covering letter format

Hi All,
applying for tier1 general dependent from india in april 2010
I will be very thankful, if some one can help me to find the documents required ( in my knowledge
1.Marriage certificate
2. funds proof
3. payslips (3months)
4. letter from the employer
5. main applicant visa copy
6. for address proof can i submit UK learners driving licence )
,covering letters and their format to apply for tier1 general dependent application from India for my wife.
I am in uk from 2006-2009 on student visa, from Nov2009 onwards Tier1 general visa.

Also add a rent/lease agreement and utility bills (if available) to this list.
