Please help for Tier 1 General Application.......

I request all the Seniors and Guru's to help me.
I am working for a IT company and also I am self employed as well.I have all the statements, salary slips , and also I got the invoices from my self employment work. I am registered as self employed and my statements show all the payments (By Cheque) made to my self employed work.
My question is what documents I need from my Accountant as he knows what I need but I still want to make sure from all the experienced and senior members who have gone through the same process.
What documents I need from the Accountant to show my earnings, my expenses and profit/loss ?
Please help me with this.

To start with,
1. Read Tier 1 (General) policy guidance *carefully*. It clearly explains the (various) documentary evidences required to be submitted for Tier 1 (General) application.
2. Use search tool to look for information already available on the forum.

Yes I checked the guidelines and also all the relevant posts in this forum for the self employed applicants.

1. Sales summary on aatnts lettterhead
2. Net income certificate on accts leterhead
3. Business accounts

Doesn't the policy guidance in para 138(v)-138(viii) and para 142 mentions clearly what documentary evidence(s) is required from self-employed applicants? And how is that different from what Sushil_ACCA has said?
