Self-Employed Letter Format

Hi

I will be soon applying for ILR under Tier-1 (General) category (with HSMP and WP). I am a self-employed for last 2+ years, have my own limited company and so far filled 2 full years of company tax returns.

I believe I need to submit a letter from accountant, I could not get a letter format, could any one help me by sharing the letter format?
